Mental Health Team

The Visiting Nurse Association Mental Health Team provides a complete, holistic method of caring for patients with a mental illness in their homes.

The Mental Health Team consists of a mental health nurse, social worker and a home care aide. The Mental Health Team cares for patients with a mental illness as a primary or secondary diagnosis. Registered nurses on our Mental Health Team are qualified to address both acute and chronic psychiatric conditions.

Members of the Mental Health Team have received instruction in effective mental health treatment. Team members caring for patients with psychiatric conditions are knowledgeable about these conditions, as well as other health conditions the patient may have. They are specifically trained to recognize changes in the patient's condition and to assist with implementing problem-solving and coping strategies.
