Wound Care Nurse Case Manager
A Wound Care Nurse Case Manager is a registered nurse who has received additional instruction and training in the treatment of wounds, skin care, and ostomy care. Routine accepted standards of wound care based on clinical and scientific research are adhered to. The Wound Care Case Manager customizes the clinical treatment, selects the appropriate products and services and anticipates patient outcomes. The information is communicated with the physician, patient and payer.
Physical Therapist
Because wounds influence a person's lifestyle and activities of daily living, the physical therapist is an important part of the Wound Care Team. The physical therapist addresses mobility status and use of adaptive equipment, as well as recommends appropriate interventions.
Certified Home Care Aide
The home care aide (HCA) provides personal care services. As the eyes and ears of the Wound Care Team, they often make frequent visits to the patient's home. The HCA assists with bathing and grooming, physical activity and exercise as instructed to help the patient achieve the desired outcome of optimal skin integrity.
Dietitian
The registered dietitian confers with patients and families to make recommendations about improved nutritional intake which can facilitate wound healing.
Patient & Family
The Wound Care Team customizes treatment based on the patient's need, wound and functional limitation. The patient and family work with the Wound Care Team for outcomes such as:
