Home Hospice Team

Hospice Team members are experts in pain and symptom control. They provide supportive counseling that strives to improve the quality of life for both the patient and their loved ones.

VNA Hospice Team members work closely with patients and families to involve them in decisions related to their care. Support and professional guidance is given to assist with coping strategies related to serious illness, separation and death.

The Hospice Team members are specially trained in caring for terminally ill patients. Members are on-call 24 hours a day to provide assistance.

The Hospice Team includes:

• Hospice Medical Director
• Patient's Personal Physician
• Nurse
• Physical, Occupational and Speech Therapists
• Dietitian
• Certified Home Care Aide
• Medical Social Worker
• Spiritual Care Counselor
• Volunteer
• Pharmacist
